LewerMark Plus CoPay Plan

The LewerMark Plus CoPay Plan is designed to meet the needs of your International Students. The plan includes a Preferred Provider Organization (PPO) as well as:

• A major medical benefit maximum of $250,000 for each injury or each sickness.
• No deductibles or lifetime maximum.
• Covered expense paid at 100% for services from a Student Health Center with no Copayment, $0.
• $15 Copayment per doctor visit.
  • 100% coverage in the PPO network.
  • 80% coverage outside the PPO network.
• $50 Copayment per hospital visit (inpatient and outpatient services)
  • 100% coverage in the PPO network.
  • 80% coverage outside the PPO network.
• Prescription drugs are covered at 100% for inpatient care or if dispensed by a Student Health Center; covered at 50% for outpatient care or if not dispensed by a Student Health Center.
• One routine mammogram and pap smear per year.
• Newborn Infant Well Baby Care
• An Out-of-Pocket Maximum of $2000 per calendar year.
• Intercollegiate Sports Benefit of $25,000 per injury.
• Unlimited Medical evacuation benefit.
• Unlimited Repatriation benefit.
• Students are covered anywhere in the world except in their home country.
